Full Range Primary Care
Phix Health brings the doctor’s office to your residents – either in their rooms/apartments or in a dedicated clinic right in your facility. Our providers perform wellness visits, diagnose and treat conditions, provide referrals, prescribe medications, and more.
Our Own Proprietary EMR
We use our own proprietary EMR to ensure the highest level of quality care. Patients are automatically evaluated for a variety of risks such as Risk of Rehospitalization, Frailty Risk, Fall Risk and our model of care is tailored to patients at various health risk levels.
If your clinical team needs to review specific patient information, we can create custom permission groups for your staff to review patient approved medical information to improve your communication with residents and their families.
Primary Care Services
Our primary care providers complete a full range of primary care services for geriatric patients. We work with a variety of medical specialties to ensure that patients receive the best care possible.
Patient Portal
Our easy to use patient portal let’s patients and their families view information on recent visits, medications, upcoming appointments and more.
Remote Patient Monitoring
For eligible patients, we provide a range of devices for each resident to take daily measurements of weight, blood pressure, blood glucose and more, depending on their needs.
Our team of clinicians will follow up with residents and notify primary care providers of any unusual readings to better manage overall health and avoid health problems before they become critical.
Chronic Care Management
For patients that are eligible, we offer an enhanced care model to manage patient care in between visits. Coupled with Remote Patient Monitoring, we can closely monitor patient health and coordinate all patient care, even with other specialists.
Medication Management
Our providers are able to prescribe your residents needed medications, including psychiatric medications and monitor health outcomes as well as evaluate potential drug interactions that might cause unwanted side effects.

How often are patients seen by the Primary Care Provider?
Several factors are taken into consideration to assess the complexity of each person’s health profile and determine the frequency of visits. They can range from monthly to quarterly for most but not all patients.

How are acute visits handled?
Our provider schedule will include open time slots for acute care follow up or other unexpected needs.
How do we receive progress notes from provider visits?
Phix Health will provide a daily record of patient visits to facility clinical team. Progress notes are available by request. With patient consent, facility clinical teams can have restricted access to our EMR to access the progress notes directly as they are completed.
What other services or programs do you offer?
For patients that qualify, we offer Chronic Care Management and Remote Patient Monitoring programs. We also offer medication management, and we have a robust network of referral partners for other services including counseling and behavioral health, home health care, palliative care and more.
